"Because he had not experienced any symptoms until adulthood, he had been affected by facial paralysis early on in the course of his illness..."

It's the "he" right after the coma that completely throws off the flow.

Can someone explain the timeline of events that occurred as directed by the given sentence. I am still confused about the answer choices D and E.
Another question is that does the structure of sentence A is Parallel ?

Roosevelt had not experienced any symptoms until adulthood- is correctly using past perfect; when he did experience symptoms, i.e after adulthood, he was effected by facial paralysis- this is an event that occurred after his adulthood and is using simple past. Option E is wrong for this very reason as it seems to invert the sequence of events.
Option D seems to suggest that because he was not effected by any symptoms until adulthood, he was effected with facial paralysis - distorts the meaning; and also the use of comma to join two independent clauses.
Also , as in MGMAT, when constructing parallel sentences, it is acceptable if the two parallel constructions use different tenses to correctly depict the sequence of events. Apart from the different form of tense used, they have a proper parallel construction "had not experienced" and "was affected ".
