Guys, how bad is it if the application was submitted without all blanks filled, including company website/phone number in work experience and reason for leaving? Some reasons for leaving have been covered in optional essay though. Is this something to bring to adcom's attention as a mistake or is it not a big deal?

A current applicant might give you a better answer but here is what I think. I am guessing you are talking about optional fields. These fields are optional for a reason. I guess the adcom doesn't require to know all these details but prefer to have them if an applicant provides them. IMO, you should've filled in the "reasons for leaving" briefly even though they are specified in the optional essay. I also think that this is not a mistake that you need to reach out to the adcom.

Finally, the adcom will reach out to you anyways if they think some information is missing from your application.

IMO, it's be better if you reached out to an adcom and convey your doubts and talk it out. That shows the adcom you're serious about your application.

FYI, the adcom at Fuqua is very helpful.

Don't want to come out as rude or pessimist, but honestly, an under-cooked application is never savory. Think of it this way: you have 1000 yumm dishes to choose from and you have to choose 100 (all are equally yummilicious and it's difficult to choose the best 100), how do you choose? Simple! You see the presentation and you see how much effort the chef has put in on presentation.

That said, if you didn't have ample time to plate up your dish, but still wanted it to be tasted, banking completely on how wonderfully the dish has been, it's completely fine. You did the best you could, right? Don't overthink or fret over it now. Relax. However, if you feel you haven't done enough, reach out to the adcoms about it. Though I wonder how the adcoms would be helpful in this regard.
