The word "the prediction" is correctly modified and the sentence is grammatically correct and well conveyed to the reader.

Clearly, the prediction is not at the heart of a black hole!

Looks good.

"a point" refers to "gravitational singularity" and not "black hole". Gravitational singularity is a concept and not a location. Hence, "a point where" is incorrect. So, choices A and D can be eliminated.

"prediction" is repeated in B, making it long and wordy.

E is passive vs C is active. Hence, answer should be C.

Meaning:
1. There is a perplexing result of General Relativity
2. That result is gravitational singularities lie at the hearts of black holes
3. At these gravitational singularities, laws of physics no longer seem to apply
4. This perplexing result of General Relativity is supported by subsequent research

The core part of the Sentence:
1. There is a perplexing result of General Relativity
2. This perplexing result of General Relativity is supported by subsequent research

Errors:
(A) One perplexing result of General Relativity is the prediction that gravitational singularities lie at the hearts of black holes, a point where the laws of physics no longer seem to apply, and that has been supported by subsequent research.

Modifier issue - A point should modify singularity but it is modifying hearts.
S-V Agreement issue in modifier - hearts/singularities are plural. A point can not describe plural hearts/ singularities
Meaning issue - It seems that a single black hole can have multiple singularities or multiple hearts


(B) One perplexing result of General Relativity is the prediction at the heart of each black hole of a gravitational singularity, a point at which the laws of physics no longer seem to apply, but a prediction that has been supported by subsequent research.

Meaning issue - This changes the meaning of the sentence and the meaning becomes illogical. it says that the prediction is at the heart of each black hole.


(C) One of the perplexing results of General Relativity is the prediction, supported by subsequent research, that at the heart of each black hole lies a gravitational singularity, a point at which the laws of physics no longer seem to apply.

Although this option corrects the modifier issue:
singularity is modified correctly by a point at which the laws of physics no longer seem to apply.
The Meaning/Core of the sentence is changed now:
1st - It says there are multiple perplexing results of General Relativity and this is one of them.
2nd - Prediction is supported by subsequent research - is a modifier now


(D) The prediction of a gravitational singularity at the heart of each black hole, a perplexing result of General Relativity but one that is supported by subsequent research, is a point where the laws of physics no longer seem to apply.

Meaning issue - This changes the meaning of the sentence and the meaning becomes illogical. it says that the prediction is a point where laws of physics no longer seem to apply. it also completely changes the meaning of the sentence.

(E) The prediction that at the heart of each black hole lies a gravitational singularity, a point at which the laws of physics no longer seem to apply, is a perplexing result of General Relativity but supported by subsequent research.

This corrects all the issues in the main sentence.
The Core Structure is maintained. The modifier is also correctly placed.
It also shows contrast correctly

Prediction happening at the heart of black hole doesn't make sense.

(D) The prediction of a gravitational singularity at the heart of each black hole, a perplexing result of General Relativity but one that is supported by subsequent research, is a point where the laws of physics no longer seem to apply. --> Incorrect. Prediction needs to be followed by that. Meaning of sentence seems to be prediction is a point where laws of physics no longer apply. This meaning doesn't make sense.

Prediction as a result of General Relativity makes sense.
