Hi folks,
What follows modifier must be the entity that is being modified here/doer of the action.
Here, "not having heard clearly" is something that is done by another person and not the speaker himself/herself. So, thinking along the same line, lets choose the best one.

(A) the speaker was asked to repeat his statement. Incorrect
(B) She asked the speaker to repeat again his statement. : Incorrect, again is redundant to repeat
(C) the speaker was asked to repeat his statement again. : Incorrect, again is redundant to repeat
(D) She asked the speaker to repeat his statement. Correct
(E) She then asked the speaker again to repeat his statement. : Incorrect, again is redundant to repeat

Not having heard clearly, the speaker was asked to repeat his statement.

Statement "Not having heard clearly"- Modifier---- Who did not hear clearly? So next part after should refer to that person.

(A) the speaker was asked to repeat his statement.---Incorrect----Modifier Error
(B) She asked the speaker to repeat again his statement.---Incorrect----Again redundant
(C) the speaker was asked to repeat his statement again.---Modifier Error
(D) She asked the speaker to repeat his statement.--Correct
(E) She then asked the speaker again to repeat his statement.----Redundancy + then not required.
