+1 for B.

A)to identify food allergies, such as --> Incorrect, "food allergies, such as milk" is nonsensical, milk is not an allergy
B)to identify food allergies, such as to --> Correct, "food allergies to milk" is idiomatic, and conveys intended meaning
C)to identify food allergies, like --> Incorrect, "food allergies, like milk" is nonsensical, milk is not an allergy
D)for idenfying food allergies, like that of --> Incorrect, "allergies of" is unidiomatic
E)for identifying food allergies, such as for --> Incorrect, "allergies for" is unidiomatic

Hence, B.

I don't think the OA is C, it has to be B for above stated reason.

IMO answer is B; with preferred parallelism and idiom usage.
