I have to be honest, this is a serious mistake and schools generally don't tend to be very forgiving in these situations. I have heard many an admissions committee team member mention this as a significant faux pas in interviews and such.
Having said that, I feel really sorry this happened to you after all the effort you put into your application. My suggestion would be to reach out to your contact on the admissions team (if you haven't been in touch, you can simply reach out to an admissions coordinator) and explain what happened honestly and succinctly. Don't over-explain or over-apologize; keep it simple and cordial. I hope they will understand and cut you some slack. My best to you for both Tuck and Yale!
