Read C: their advocacy   for ordinances that mandate fees on plastic containers prevent   -- it should be prevents because advocacy is singular.

"that " is actually not referring to anything. even if we look for nearest noun(ordinances) it makes no sense at all.
 
no pronoun error is removed and meaning is correct.
rest of the choices are easy to remove.

B mandating fees on plastic containers is preventing -hold
C that mandate fees on plastic containers prevent- that is unnecessary
D to mandate fees on plastic containers are preventing - again SVA error.
E to mandate fees on plastic containers is preventing- hold
Between B and E, the infinitive form is unnecessary as it is used when we need to present the purpose or the intention of an action. This phrase is actually a shorter way of “writing” “in order to verb”. Not the case here hence B.
Hence ans is B

We are using "mandate" out here which is subjunctive verb. And subjunctive form comes in below form only,

Subjunctive clause+ that + S+V+O
So, out here usage of that is mandatory.
Under same option 1 should be only correct answer as that only follows this standard.
Would someone pls elaborate why we have diverted from very standard rule.

C has a clear SV agreement problem, ‘advocacy; is singular but ‘prevent’ is plural. ‘Fees be on’ is very awkwardly constructed. So A should not be considered. D has SV agreement problem too, ‘advocacy’ is singular ‘are’ is plural. ‘to mandate’ in E is improperly constructed, so B is the right answer.

The phrase "mandating fees on plastic containers" is a noun modifier, so a comma would not be appropriate. (Comma + -ing typically indicates an adverbial modifier.

The meaning here is that the advocacy is preventing litter. Which advocacy? The advocacy for ordinances. Which ordinances? The ordinances mandating fees on plastic containers.

A and C get the verb wrong ("advocacy . . . prevent"). It also doesn't make much sense to say "mandate that fees be on plastic containers."
D and E suggest that the ordinances exist in order to mandate fees. It makes more sense to simply say that the ordinances *do* mandate fees. It's not something the ordinances are going to *try* to do.

 When we first read the sentence, we're confused whether the organizers' advocacy or the fees on plastic containers is what's "prevent  significant amounts of litter." That confusion means that the sentence as written, choice (A), does not work. 
How do we make the sentence clear, then? Let's figure out what we want to make it say first. If the community organizers want to take responsibility, or credit, for the lower amount of plastic waste, then they want to claim that "their advocacy" prevented amounts of litter. So, we need to make "prevent" the verb of "their advocacy," and only (B) and (E) match that singular subject. 
The main difference between (B) and (E) is tense: (E)'s "to mandate" would mean that these fees don't yet exist, but they do because the community organizers are taking credit for some of the early effects of these fees. Choice (B)'s "mandating" correctly indicates that these fees have already been put into effect. 
  The correct answer is Choice (B)  .
