(B) vitamin supplements that have the potential to lead to potential to lead to ; verbose

(D) vitamin supplements, a practice that can, potentially, lead to meaning issue

OPTION C is correct

Some things to note:
1. "potential for kidney damage" and "potential to lead to kidney damage" are commonly used forms of idioms.
2. do the vitamins in any dose themselves have kidney damage potential or the practice of eating higher than recommended dose does? the practice causes kidney damage -- in correct amount the vitamins do not cause damage.

Based on this, D seems to be the right choice...
(A) vitamin supplements that have the potential of leading to
(B) vitamin supplements that have the potential to lead to
(C) vitamin supplements that potentially can lead to
(D) vitamin supplements, a practice that can, potentially, lead to
(E) vitamin supplements, a practice that has the potential for leading to

Split 1: Meaning
Does vitamin supplements per se cause kidney damage? No, it is the practice of consuming excess vitamin supplements. Hence, A, B and C are out for meaning issue.

Split 2: Can and potential used together are redundant
Split 3: lead to vs leading to
"lead to" is preferred over "leading to" per this sentence as it is clear, concise and direct. Though, choice D contains redundancy, it is grammatical and hence is the answer.

In the original sentence we have the meaning issue. Actually, not vitamin supplements have the potential of disease, but overcompensating by imbibing is the reason. Hence, we can eliminate answers A, B ,and C.

(C) vitamin supplements that potentially can lead to
(D) vitamin supplements, a practice that can, potentially, lead to This choise corrects the meaning issue

Breaking down the base sentence:

Patients diagnosed with vitamin deficiency overcompensate for this by taking more than medically recommended amounts of vitamins.
The modifier - This act could lead to kidney damage

The modifier is modifying the act of taking excess vitamins

A, B, and C mean taking excess vitamins that could lead to kidney damage. It's not the vitamins that will lead to kidney damage, but the excess consumption of vitamins.

potentially means could or could not and potential for mean what it takes to
potentially makes more sense in this context.
