Thanks for your question. You're right in that you'll have to disclose all of your academic history, so this will be all about how you frame your story. The good news? It looks like you learned something since your first couple semesters and that you got yourself back on track. Top MBA programs will want to hear you tell them what changed. That's where a lot of prospective students need help. It's not going to be about making excuses of why your grades were bad, nor will it be about turning your weakness into a strength. It will be about crafting a compelling story around the catalyst for change and how it affected every aspect of your life.
The tier of school all depends on the rigor of your current job, career trajectory (promotions?), leadership capacity, community involvement, and of course the GMAT. Taking a step further, the specific schools you should apply to will largely depend on your goals and what you hope to get out of your MBA experience. Based on what you shared so far, I'd aim for tier 2 schools (outside the top 15) with 1-2 tier 1 stretch goals.
