The meaning is, medical issues are tough to solve because there are no testimonies from other doctors against the defendant.
Suits is plural and hence will require either a plural object form later (doctors).

Medical malpractice suits can be difficult to prosecute because often there is a lack of another doctor to testify against the defendant.
A. there is a lack of another doctor to testify against - Improper usage of singular doctor against plural suits
B. they lack some other doctor’s testimony against - They usage is incorrect (can not be used for suits)
C. other doctors refuse to testify against - Precise and correct usage.
D. another doctor will have refused to testify against - Same as option A
