I took the GMAT back in 2016 and got a 750. I ended up having to go to rehab instead of grad school. (Long story). I'm doing better, actually working at a rehab myself, but I was thinking of going back now. Do I have to take the GMAT again though? Do the scores expire? If I do have to take it again, has the test changed much since 2016?

Your score will be valid until 2021. (5 Years validity starting from test day)

While the test has changed a bit, this should be of no concern to you as your impressive score is still valid for the next two application seasons.
