Its simple. According to the gmat whenever the sentence indicates intent we need to use the infinitive form to+verb not verb-ing.
For example
The investors sold their stock rapildy, causing panic
Here the sentence indicates no intent from the investors to create panic but their act of selling their stocks rapidly did in fact create panic.
Lets look at the inifinitive form now.
The investors sold their stocks rapidly to create panic
Here the sentence indicates intent from the investors. Here their intent was to create panic. But in this sentence we do not know if they were successful in creating panic or not. Just that they wanted to.

Ok - this is a parallelism question, but you have to be careful to make sure you are looking at the correct elements to be made parallel.
This question is presenting a three item list that needs to be in this format to be correct "X, Y, and Z": to revamp..., institute..., and create. Note that with infinitive parallelism we can drop the "to" before the 2nd & 3rd items in the list. Interestingly, answer choices A, B, and C have an improper structure of "X, Y, Z" - no "and". We need an "and" somewhere in the list to have an actual sentence*. That eliminates A, B, and C without having to deal with participle vs. infinitive in the parallel structure.
Looking at D and E, there is an idiom choice (you could view it as a meaning choice) between "for taking" and "to take". Looking at the context of the sentence, we need to use "to take" to generate the proper meaning that the non-government organization (looks like the word "organization" was inadvertently left off choices D&E) will be the thing that actively "takes" charge.

it should be,

Please note that the un-underlined part lists the infinitives ‘to revamp /, institute’ without a conjunction in between, implying that another infinitive factor is ensuing to maintain parallelism. In the context, ‘to create’ is indeed the third factor, which the NSA has urged. Therefore, participial use of ‘creating’ is wrong in the given case. The real choice should fall between D and E.
Since a purpose ‘to take charge’ is indicated in the text, the best diction to denote such a purpose is the infinitive. Therefore, D drops out and E is the eventual option.

why do they want to a create a sp organisation ? they must have some solid rationale / some intent..........
INTENT : if this is the word then INFINITIVES who are always left behind, who always loom in the dark in gmatland...... ....cannot afford to miss this golden opurtunity and rush in at a mad pace to fill the vaccum.

It`s a parallelism problem. to revamp....to institure.......to create......Thus eliminate A,B,C.Between D and E , E is the correct answer.Because its to take not for take.

@mun23, I am afraid I will have to correct you here. While you are right in eliminating A,B,C on grounds of lack of parallelism, your reason to eliminate D is wrong.
I am mistaken. It seems "for taking" cannot be used to define a purpose. When you want to highlight the intention/purpose then we have to use to take. Otherwise there is no error with the usage of "for taking"

Hi,
The right answer is E. You need to list down the items and then selects the Parallel constructions. Easy way is go by POE.
to revamp is in parallel with to take. So B,C,D are wrong.
The sentence has "Has urged" so creating is wrong as it represents continues tense. So A goes off.
E is the answer.

In option B the modifier that takes.... refers to nongovernment organization . The meaning conveyed is:
Nongovernment organization takes charge of computer security planning.
The simple present tense is wrong - it implies that the special nongovernment organization already exists, and it generally takes control of computer security planning. However this is not the case - the special nongovernment organization is yet to be created. An action in future cannot be depicted by simple present tense.
The conditional tense would take control should be used to indicate a possibility, not a simple present tense.
In correct option E, the intended meaning is as follows:
National Academy of Sciences urged the nation to perform 3 things:
1. revamp procedures
2. institute response teams
3. create a special organization.
Why NAS has urged the 3rd action -what is the purpose? ... to take charge of the security planning. Whenever an intent or purpose is expressed it is preferred that the infinitive form to+verb be used.
The word "organization" is not present in the last two options. Is this a mistake or really the case?
If not, can we afford not to have word "organization" in the options and still consider the options correct? Please throw some light?

This is a typographical error. In the official guide, the term "organization" is there in options D and E as well.

The correct choice is written below.

In the above sentence, to has been used before the verb revamp, institute, create because the tag is and. So, we can rewrite the correct sentence as follows:
Warning that computers in the United States are not secure, the National Academy of Sciences has urged the nation to revamp computer security procedures, to institute new emergency response teams, and to create a special nongovernment organization to take charge of computer security planning.
So, here is the question: Why National Academy of Sciences has urged the nation?
The simple answer is: to do blah blah blah. This to do is NOT a verb;to do is infinitive (to+verb). The infinitive (to+verb) can't be verb anymore. So, revamp, institute, and create are NOT verbs in this sentence! The verb is urged.
Here is the correct choice again.
Warning that computers in the United States are not secure, the National Academy of Sciences has urged the nation to revamp computer security procedures, institute new emergency response teams, and create a special nongovernment organization to take charge of computer security planning.
In the above sentence, to take is an infinitive. So, it can't be verb anymore. Most importantly, we never express our purpose/intention of an action without any verb (like urged in this sentence) in the sentence. In the sentence, something is done to take ''charge of computer security planning''. egmat says: It is clear that taking charge of computer security planning is the purpose for creating this special nongovernment organization. My question is to egmat: this something is done is what, actually? I mean: Which one (from the below) can be replaced with something is done? is it:
1/ to create (infinitive) a special nongovernment organization?
or,
2/ National Academy of Sciences has urged (not infinitive at all-this is the main verb) the nation?
I think, to take charge of computer security planning CAN'T be the purpose of to create a special nongovernment organization. Because, to create is the infinitive itself!

The reason that "and" is before "create" seems to be that "to take charge" modifies either "to create" or "organization."
The truth is that the wording is not optimal. "create an organization to take charge" does not clearly convey any particular meaning. Is the organization going to take charge or is "to take charge" the purpose of creating an organization.
I think something along the lines of "create an organization that will take charge" would more clearly express the meaning.
Meanwhile, you said the following:

What you said does not really make sense, You could have an infinitive stating the purpose of an infinitive. Consider the following simple example:
I urged John to run quickly to catch the driver before he left.
