Hi guys , how to obtain copy of official score report as NUS application has option to upload the score report? Is it mandatory even after sending the score report to NUS?

Would be great to receive your inputs on below:
1. The R2 deadline is 15th January. This is the deadline for stage 2 of the application right? Stage 3: Video Assessment can be submitted post the 15th January deadline?
2. Is there a particular format for CV for the application? No such template is attached on the portal under the "Supporting Documents" section.
3. In "Financial Support Documents" section, the instructions say "recent pay slip". Typically, pay slip of any past 3 months works but please confirm since I haven't yet received the December pay slip from my organisation and plan to upload the November pay slip.
