Pretty straight forward, question is testing simple idiom - not only x but also y.

Here are my reasons to eliminate each option:

An increasing body of scientific research suggests that the overall health of the human microbiome – consisting of as many as 1000 different species of bacteria in the gut alone – is not only influenced by diet and alcohol consumption, but also are by behavioral factors as diverse as insomnia, exercise, and oral hygiene.

(A) not only influenced by diet and alcohol consumption, but also are by
not parallel, we have "not only influenced ....but also are ..." verb in X and only helping verb "are" in Y are not parallel

This one is about parallelism.

An increasing body of scientific research suggests that the overall health of the human microbiome – consisting of as many as 1000 different species of bacteria in the gut alone – is not only influenced by diet and alcohol consumption, but also are by behavioral factors as diverse as insomnia, exercise, and oral hygiene.

Error Analysis

Here we have to notice that the correct idiom is not only x but also y, so entities after these markers must be parallel; here in the original sentence we have not only influenced,, but also are, are is not parallel with influenced so we need one option in which both entities are parallel.

(A) not only influenced by diet and alcohol consumption, but also are by -wrong per error analysis
(B) not only influenced by diet and alcohol consumption, but also -wrong per error analysis
(C) influenced not only by diet and alcohol consumption, but also are by -not only by..., but also are by... "are" is making the second part not parallel
(D) influenced not only by diet and alcohol consumption, but also by -Seems good
(E) influenced not only by diet and alcohol consumption, but also -Here the second parts needs a by, since the first part is [b]not only by..[/b]

So (D) is our answer.

Here after "not only" goes "by", this implies that after "but also" should be "by" too, without any verbs

It goes.. Not only X but also Y. X and Y should similar and comparable elements to make the structure parallel(if X is a verb, Y is a verb, if X is an adjective, Y is an adjective, etc)

Doesn't maintain the parallel structure explained above and also use "are" to describe singular noun microbiome - Eliminate

Correct Parallelism is what we want to test our answer choices on. In the construction not only A but also B, the elements of the parallelism A and B must be not only structurally identical, but also logically similar (Did you notice how I’ve just used this idiom? :-D).

1. The following structures are parallel:

I. Iphones are produced in NOT ONLY China BUT ALSO India.
NOT ONLY noun BUT ALSO noun. Correct

II. Iphones are produced NOT ONLY in China BUT ALSO in India
NOT ONLY prepositional phrase BUT ALSO prepositional phrase. Correct

III. Iphones’ processors are NOT ONLY designed BUT ALSO produced in California.
NOT ONLY verb BUT ALSO verb. Correct

2. The following structures contain incorrect parallelism:

For the sake of convenience and speed, let’s use just health instead of the overall health of the human microbiome, and get rid of an appositive modifying microbiome for a moment. Keeping all the above in mind let’s analyze answer choices.

A. health is not only influenced by diet and alcohol consumption, but also are by behavioral factors

In short: NOT ONLY working verb BUT ALSO linking verb… we know that working verb is not parallel to linking verb.

Additionally, the second element of the parallelism doesn’t make any sense with the root phrase: health is are by behavioral factors… that’s nonsensical and thus A is out.

B. health is not only influenced by diet and alcohol consumption, but also behavioral factors

In short: NOT ONLY verb BUT ALSO noun… we know that verb is not parallel to noun.

Additionally, the second element of the parallelism doesn’t make any sense with the root phrase: health is behavioral factors… that’s nonsensical and thus B is out.

C. health is influenced not only by diet and alcohol consumption, but also are by behavioral factors

In short: NOT ONLY prepositional phrase BUT ALSO linking verb… we know that prepositional phrase is not parallel to a linking verb.

D. health is influenced not only by diet and alcohol consumption, but also by behavioral factors

In short: NOT ONLY prepositional phrase BUT ALSO prepositional phrase… finally we have a correct parallelism.

Additionally, the second element of the parallelism DOES make sense with the root phrase: health is influenced by behavioral factors… that’s the correct solution.
